Jen’s MSc News!

Congratulations to Dr Jen Blair, our co-director, who graduated from Nottingham with an MSc in Applied Ergonomics and Human Factors, with Distinction!

Jen said “I am so relieved to have finished and really pleased to have been awarded a distinction, the icing on the cake! Doing a Masters is never easy, but this was made more challenging by a few personal issues. I am glad I stuck at it though. I’ve learnt so much about the science of human factors and how relevant this is, not just to healthcare but to many other industries. In fact, studying human factors in this broad way has helped me identify areas where healthcare has so much to learn from other industries, such as designing for fatigue. Onwards and upwards!”.

“Should I insist on a full written report after AAR, Swarms and MDTs? If I don’t, how can I capture the learning?”.

Our thoughts:

There is no national requirement to generate reports or forms after the other learning responses (AAR, Swarm, MDT), the requirement exists only for PSII reports. However, it is important to capture any immediate mitigations or safety actions taken and also what was learnt from the discussions.

For AAR or Swarm some organisations are using Datix to record key learning points, this has the advantage of minimising additional paperwork and allows for organisational summarising or theming of findings from similar events.

For an MDT a similar approach could be taken, even where similar incidents or a related theme is considered, as incidents can be linked or clustered. Documenting key learning from an MDT within Datix would provide valuable centralised information and would support the transition to improvement work in the relevant areas. We think an MDT proforma approach would be best placed here, to standardise findings and to emphasise important aspects like prompting the recording of important interactions / barriers / facilitators and suggested areas for improvement.
