Using System-based debriefing to facilitate compassionate learning responses

A recent NHSE snapshot survey of Patient Safety Incident Response Framework PSIRF learning response methods found the most frequently used is AAR. However a lack of understanding about AAR was noted, with the need for more training and quality assurances in place.

Being Human in Healthcare Ltd have developed and recently published the Systems After Action Review SAAR model in the The Ergonomist | CIEHF that can support a more psychologically safe, systems-based AAR debrief. Our one-day course teaches staff to apply both the Being Human SAAR model and Swarm Guide to patient safety events, to ensure more high quality, psychologically safe, system-focused debriefs. This interactive course is underpinned by interprofessional education and debriefing concepts and supports participant learning through practical application and constructive peer and facilitator feedback.
